In compliance and consideration for the use of the facilities and participation in the events and activities of the Shooting Range:

I understand that there are inherent risks and hazards involved in shooting sports. The danger as much as policies exist and cannot be eliminated, such as:

   •  Injury, damage, or death due to accidental shooting.

   •  Injury, damage to property, or death due to ricochets from targets or projectiles.

   •  Ear injury due to shots fired.

I understand the necessity for the use of eye and ear protection during the activity and by which I shall abide to wear such protective equipment at all times while within the premises.

I hereby declare that I have had the opportunity to discuss with the facility representative the risks and hazards detailed above and the dangers of the sport. I declare that I have sufficient knowledge and skills in the handling and care of a firearm. 

I hereby waive all claims and release and hold harmless the facility, its owners, directors, employees, representatives, agents, and volunteers, from any and all damages, expenses, liabilities, claims, lawsuits, fees, causes of action as a result of injury, damage to property or death to myself, heirs, family members which may arise during participation or presence in the venue of the event

I am fully aware that the facility does not employ any medical personnel that can provide emergency medical aid whenever an incident arises.

I hereby release and covenant not to sue the facility, its owners, directors, employees, representatives, agents, and volunteers, from any and all claims in the occurrence of any injury, damage, or death while participating in the activity or event of the facility.

I further declare that:

   1.  I am not under the influence of alcohol or drugs;

   2.  I am fully aware of my mental faculties;

   3.  I have not been denied of any purchase of a firearm by lawful means;

   4.  I am permitted by law to possess and handle a firearm;

   5.  I have not been convicted of a felony involving moral turpitude or domestic violence;

I sign this form with full knowledge and understanding of the contents herein. I hereby declare that my participation in this activity is purely voluntary and without any inducement of consideration or coercion. I have full knowledge and assume all the risks and dangers involved in the activity that I may sustain or incur during the activity, whether as a participant or as a spectator.

Registration for this event is $60 per participant.

A portion of the proceeds will be donated to the ONE TRIBE FOUNDATION (Formerly 22Kill) to aid in veteran's suicide prevention.
